Diritto alla salute, l'Italia prenda esempio dai nuovi progetti in Senegal e Sudan

La cacciata dei lavoratori poveri e stranieri da Rosarno ha sollecitato in molti di noi riflessioni che vanno al di là di quell’episodio. Abbiamo compreso che i sentimenti di paura, disprezzo e arroganza per chi è diverso (perché è povero, ha la pelle di un altro colore, parla un’altra lingua) nutrono l’anima di molti italiani e conducono troppo spesso ad atti barbarici. Abbiamo compreso che razzismo, xenofobia, insofferenza per il povero, paura per lo sconosciuto sono sentimenti diffusi, che rendono più inospitale la città, più piccolo, insicuro, misero il mondo di ciascuno di noi.
É anche per questo che mi hanno colpito due episodi avvenuti in Africa, che hanno a che fare col mio mestiere di urbanista e che in queste settimane sono stati riportati alla mia attenzione. Mi riferisco a due strutture sanitarie realizzate da italiani l’una a Ovest l’altra a Est nel Continente nero.
Un ospedale in Senegal, a Kaedi. Costruito secondo un modello completamente diverso da quello occidentale, dove le degenze sono realizzate per ospitare solo il paziente e consentire pochi visitatori. Lì hanno constatato che la malattia è vissuta in modo molto diverso che in Europa. Se un membro della famiglia si ammala tutta la famiglia lo segue, lo assiste, cucina per lui, gli fa costantemente compagnia. Allora a Kaedi l’architetto Fabrizio Carola e i sanitari hanno organizzato una serie di padiglioni circolari, realizzati con materiali poveri e locali e maestranze del posto, disposti secondo tre anelli concentrici. Nei padiglioni dell’anello centrale, tutti i servizi sanitari: le sale operatorie, quelle per le medicazioni, i medici e gli infermieri, i laboratori ecc.; nell’anello intermedio le degenze, destinate ai pazienti; quelli esterni sono per le famiglie. Dei collegamenti tra i padiglioni dei diversi anelli costituiscono le comunicazioni senza interferenze tra i percorsi del personale sanitario e quello delle famiglie. Convivenza e assistenza da un lato, igiene e presenza sanitaria dall’altro sono entrambe garantite. Il rispetto per una cultura diversa e l’attenzione alle risorse locali e a possibilità innovative del loro uso hanno sostituito l’imposizione di modelli calati dall’alto.
Altrettanto significativo l’altro esempio: la realizzazione di un Centro Salam di cardiochirurgia, di elevatissimo livello specialistico, nel deserto del Sudan vicino a Khartoum, realizzato da Emergency con un’equipe di medici e architetti italiani. Anche qui, nella progettazione del complesso, massima attenzione alle risorse e alle abitudini locali: un’architettura solidamente legata al luogo, realizzata con materiali semplici e con largo impiego delle tradizioni e dei materiali locali. E la formazione di personale medico e infermieristico locale, capace di insegnare a sua volta.
Ma anche qualcosa in più: una grande lezione di uguaglianza. Essa emerge dalle parole d Gino Strada (nella prefazione del libro dedicato dall’architetto, Raul Pantaleo, al diario dell’esperienza di cantiere: “Attenti all’uomo bianco”, edizioni Eleuthera, Milano 2007). Qualcuno ha definito il centro Salam una “cattedrale nel deserto”, perché si tratta “di una struttura di assoluta eccellenza, perfino innovativa per molti aspetti, dove praticare una medicina difficile e complessa, che richiede grande impegno di risorse umane ed economiche”. Meglio costruire strutture elementari, ambulatori e dispensari, cliniche di base.
“Noi ci siamo rifiutati di mettere le due scelte in alternativa”, risponde Strada. Alla periferia di Khartoum c’è un centro pediatrico di Emergency che già si occupa della medicina di base. “Ma abbiamo voluto andare oltre. Ci siamo convinti che anche in quei luoghi, dove le poche cure mediche disponibili sono solo a pagamento, sia necessario affermare il diritto a essere curati quando si è feriti o ammalati. Un diritto naturale prima ancora che un solenne principio […] Così si è radicata in noi la convinzione che curare gli esseri umani non possa essere un’attività discriminatoria, solo i neri e non i bianchi, solo le donne e non gli uomini, solo i poveri e non i ricchi”. Curare deve essere un gesto che riconosce a tutti i pazienti (pazienti, non clienti) eguaglianza in dignità e in diritti. “Non possono esistere, nella medicina, pazienti di prima e di seconda classe, cure per i ricchi e cure per i poveri del mondo. Tutti hanno diritto a cure di alto livello e gratuite”. Una lezione. Saremo capaci di seguirla nel nostro paese?

Even when the British ended their occupation of Egypt in 1936 (with the exception of the Suez Canal Zone), they maintained their forces in Sudan. Successive governments in Cairo, repeatedly declaring their abrogation of the condominium agreement, declared the British presence in Sudan to be illegitimate, and insisted on full British recognition of King Farouk as King of Egypt and Sudan, a recognition which the British were loath to grant. It was the Egyptian Revolution of 1952 which finally set a series of events in motion which would eventually end the British occupation of Sudan. Having abolished the monarchy in 1953, Egypt's new leaders, Muhammad Naguib, who was raised as a child of an Egyptian army officer in Sudan, and Gamal Abdel Nasser, believed the only way to end British domination in Sudan was for Egypt itself to officially abandon its sovereignty over Sudan. Since Britain's own claim to control in Sudan theoretically depended upon Egyptian sovereignty, the revolutionaries calculated that this tactic would leave Britain with no option but to withdraw. Their calculation proved to be correct, and in 1954 the governments of Egypt and Britain signed a treaty guaranteeing Sudanese independence. On January 1, 1956, the date agreed between the Egyptian and British governments, Sudan became an independent sovereign state, ending its nearly 136 year union with Egypt and 55 year rule by the British.

Tewfik's acquiescence to British occupation as the price for securing the monarchy was deeply detested throughout Egypt and Sudan. With the bulk of British forces stationed in northern Egypt, protecting Cairo, Alexandria, and the Suez Canal, opposition to Tewfik and his European protectors was stymied in Egypt. In contrast, the British military presence in Sudan was comparatively limited and eventually revolt broke out. The rebellion in Sudan, led by the Sudanese religious leader Muhammad ibn Abdalla, the self-proclaimed Mahdi (Guided One), was both political and religious. Abdalla wished not only to expel the British, but to overthrow the monarchy, viewed as secular and Western-leaning, and replace it with a pure Islamic government. Whilst primarily a Sudanese figure, Abdalla even attracted the support of some Egyptian nationalists and caught Tewfik and the British off-guard. The revolt culminated in the fall of Khartoum and the death of the British General Charles George Gordon (Gordon of Khartoum) in 1885. Tewfik's forces and those of the United Kingdom were forced to withdraw from almost all of Sudan with Abdalla establishing a theocratic state.
Abdalla's religious government imposed traditional Islamic laws upon Sudan and stressed the need to continue the armed struggle until the British had been completely expelled from the country and all of Egypt and Sudan was under his Mahdiya. Though he died six months after the fall of Khartoum, Abdalla's call was fully echoed by his successor, Abdallahi ibn Muhammad who invaded Ethiopia in 1887, penetrating as far as Gondar, and the remainder of northern Sudan and Egypt in 1889. This invasion was halted by Tewfik's forces, and was followed later by withdrawal from Ethiopia.

If parties have the political resolve it won't take more than two weeks as we have discussed everything and what remains is to take decisions," he said.Sudanese government officials and rebel groups present in Doha have not been yet involved in direct talks, Omar told reporters in the Qatari capital.Instead consultations are being held with mediators from the United Nations, African Union and the host nation, he added.Omar, a member of the official government delegation, said Sudan is prepared to talk with any of the four rebel groups that is ready for negotiations.Representatives from the Justice and Equality Movement (JEM), the main rebel group, on Saturday held consultations with mediators to resolve "questions of procedure" before direct negotiations.Darfur rebels had two rounds of talks with Khartoum government officials in Qatar -- in February and May 2009.But other factions have refused to join the talks in Doha and the JEM later said there is no point in taking part if there is no unity among the rebels.Rebels and government officials were also due to meet in November, but the talks failed to materialise.The United Nations says up to 300,000 people have died and 2.7 million fled their homes since the ethnic minority rebels in Darfur first rose up against the Arab-dominated Sudan government in February 2003.Khartoum says 10,000 people have been killed.Sudan is due to hold a presidential and parliamentary election in April, the first polls in 24 years.
